Does Caffeine Trigger Migraine Attack? A Smartphone Headache Diary Study

Background: Migraine is common neurological condition in developed countries. The role of caffeine in migraine has been not established yet. Mobile applications are more useful than paper diary because they can increase patient compliance. Therefore, we tried to investigate the relation between intake of caffeine and the occurrence of
a migraine attack using a smartphone electronic headache diary.
Methods: We enrolled 77 patients with migraine and conducted a prospective cross-sectional study. We used the
smartphone headache diary to capture the details regarding caffeine intake, and headache occurrence. We investigated
the association between caffeine intake and headache occurrence at every day based on daily records of
smartphone headache diary.
Results: We analyzed the data of 49 patients who completed more than 16 attacks of headache. Headaches have
occurred in average of 24.1 during application working day. The mean duration of recoding headache diary
was 181.1¡¾126.2 days, and the average number of days in which the patient actually wrote the headache diary
was 118.4¡¾108.2 days. Caffeine intake was significantly associated with migraine attack (OR for migraine attack=
1.4, 95% CI, 1.2-1.6, p<0.001) after adjustment of age and sex. Among migraine attacks with moderate-to-severe
headache intensity, migraine attacks were significantly associated with caffeine intake after adjusting covariates
(OR=1.4, 95% CI, 1.1-1.7, p=0.002).
Conclusion: Caffeine intake was significantly associated with headache attack among migraineurs. Smartphone
headache diary application is an effective tool to assess and study migraine.
